What is this service for?

You may be asked for a fit or sick note from your employer if you've been off work for health reasons.

Self-certification

If you've been sick for more than 4 days in a row, but less than 7, you can self-certify your illness using a SC2 form. You can access this form from your employer or on the HMRC website.

Statement of fitness for work

Your GP will provide you with a statement of fitness for work (fit note) if you're not well enough to work. This is usually requested as evidence to support payments of Statutory Sick Pay (SSP).

If you've been admitted to hospital

Sick notes can be issued by hospitals to cover the period of time you were admitted. Once discharged your GP can issue a sick note based on your hospital discharge letter.
