Our diabetes clinic opening times have changed. Please ensure you check these new times before visiting our practice.
To attend our clinic, please ensure you have registered with our practice. To register, please complete our registration form.
New patient registration
Registration forms are available for collection at the practice.
The practice will then contact you to arrange a voluntary patient check-up.
Unable to attend your appointment
If you're unable to attend your appointment, please contact the practice on 0123 456 782.
Interpreting services
Patients are able to access the telephone interpreting service to contact any service within NHS Greater Glasgow and Clyde.
Emergency appointments
If you require an emergency appointment, please phone the practice on 0123 456 782 at 08:00. If the practice closed, phone 111.
Home visits
Contact the practice before 10:00 to arrange a home visit.
You may request a home visit if you are housebound or too ill to visit the practice.
